Please complete the new patient form and bring it to your appointment along with your driver’s license, insurance cards and list of current medications. If applicable, also bring your specialist physician co-pay.
If you have had testing done at another facility other than Clarion Hospital that relates to your visit with us, it is important that we have this information on or before the day of your appointment. This includes lab work, scans, and x-rays. If you do not, it could result in your appointment being rescheduled until the records are obtained. Most often, reports are sufficient and can be faxed to us at 814-226-8141. If you have had an MRI, mammograms, or PET scan please bring the CD of the testing along with the typed report. This can be obtained from the facility that performed it.
